Header showing in sidebar…

I’m having an odd issue on my contact page and I’m at a total loss. My header is showing in the sidebar as if it’s a mobile responsive “box” with the header info in it and a 404 error. Can anyone help me out here? It’s hard to explain, but you can view the problem at: http://maas.northandmorello.com/contact-us/

I have my page set to a Contact Page format, and I have deleted ALL widgets that were anywhere in any sidebar/footer/etc.

If I try my page as a Default Format, it doubles my contact form but my “header box” disappears.

Hi Kathleen,

Kindly go to Appearance>>Theme Option>>Footer

You will find the option for map option there. Add only map URL there.
